Testosterone — What It Is and Why It Matters

Testosterone is the primary male sex hormone, but its functions extend well beyond reproduction. It governs muscle mass and bone density, fat distribution, red blood cell production, energy levels, mood, sleep quality and libido. In short, it is one of the body's central metabolic regulators.

Testosterone is produced mainly in the testes, under the control of the pituitary gland via LH (luteinizing hormone). This is a complex feedback loop: the brain senses how much hormone is circulating and adjusts production accordingly.

Three Key Markers: Total, Free and SHBG

A testosterone blood test can return several numbers:

Total testosterone — the amount of hormone in the blood overall. This is the main measurement, but not all of it is biologically active.

SHBG (sex hormone-binding globulin) — a protein that binds testosterone and prevents it from entering cells. The more SHBG present, the less free, biologically active testosterone is available.

Free testosterone — the unbound fraction that can enter cells and act. A person can have a normal total testosterone but an elevated SHBG — and feel as though they have a deficiency.

This is why a doctor needs to see the full trio of markers, not just one number.

Age and Natural Decline

Testosterone peaks at around age 20–25, then naturally begins to fall — roughly 1–2% per year from the age of 30–35. This is a normal process sometimes called andropause. The decline can be faster, however, when additional factors are at play — obesity, chronic stress, illness or poor sleep.

Lifestyle Factors That Drive Testosterone Levels

This hormone is highly sensitive to lifestyle. That is actually good news: meaningful change is possible without medical intervention.

Sleep is probably the single most important factor. Most testosterone is released during sleep, especially in deep and REM phases. A landmark study showed that in healthy young men, just one week of sleeping only 5 hours per night reduced testosterone by 10–15% (JAMA, 2011). Seven to nine hours of quality sleep is not a luxury — it is a physiological requirement.

Body weight — obesity is strongly associated with lower testosterone. Adipose tissue, particularly in the abdominal area, produces the enzyme aromatase, which converts testosterone into estrogen. Obesity also raises SHBG and promotes chronic inflammation. Research shows that weight loss can significantly increase testosterone levels.

Physical activity — especially resistance training. Intense strength exercise triggers a short-term testosterone spike after a workout. The long-term hormonal effect is modest, but staying active helps maintain muscle mass and healthy weight — both linked to better testosterone profiles.

Stress and cortisol — chronic stress raises cortisol, which suppresses testosterone production. Stress management — through mindfulness, time in nature, social connection — is not a soft recommendation: it reflects a direct physiological mechanism.

Alcohol — regular consumption reduces testosterone through several pathways: it directly suppresses testicular function and impairs liver metabolism (the liver plays a key role in hormone processing).

Diet — very low-fat diets may lower testosterone because steroid hormones are synthesized from cholesterol. A balanced diet with adequate healthy fats (especially unsaturated) and zinc-rich foods (shellfish, seeds, legumes) supports normal production.

When It Is Worth Getting Tested

Consider testing if you experience: persistent unexplained fatigue, loss of muscle mass and strength, accumulating abdominal fat, persistently low mood or lack of motivation, sleep problems, or reduced libido — particularly if several of these symptoms appear together.

The test is most informative when taken in the morning (7–10 am), fasting — testosterone follows a strong daily rhythm and can be 20–25% lower by the afternoon.

What Low Testosterone Does Not Explain

Low testosterone is not the culprit behind every problem. Fatigue, low mood and abdominal fat can have many other causes — thyroid issues, poor sleep, chronic stress, or vitamin D deficiency. A blood test provides one data point. Clinical context always matters.
