AST — Not Just the Liver

AST (aspartate aminotransferase) is found not only in the liver but also in muscles, the heart and the kidneys. An elevated AST therefore does not always point to a liver problem — intense exercise, muscle injury or a heart attack can all raise AST.

When both ALT and AST are elevated together, that is a stronger signal that the liver is the source. The AST/ALT ratio can help doctors make an initial distinction: alcoholic liver disease typically produces an AST/ALT ratio above 2.

GGT — The Alcohol and Bile Duct Marker

GGT (gamma-glutamyltransferase) is the most sensitive alcohol indicator among liver enzymes — it can rise even with modest but regular alcohol intake. It also rises with cholestasis, when bile has difficulty draining from the liver into the intestine.

When GGT is elevated alongside raised bilirubin or alkaline phosphatase, bile duct disease becomes a concern.

The first step is not to self-diagnose. A single mildly elevated result can be linked to a temporary factor: a recent infection, medications (even over-the-counter ones), or intense exercise. Doctors typically repeat the test a few weeks later and, if necessary, order additional tests — a liver ultrasound, viral hepatitis screening.

That said, there are things you can do right now.

Lifestyle Changes That Help the Liver

Body weight is the most powerful lever in fatty liver disease. Research shows that losing just 5–10% of body weight significantly reduces ALT and improves liver histology.

Physical activity does more than help with weight loss — it directly reduces liver fat. Even 150 minutes of moderate-intensity aerobic exercise per week produces a visible effect.

Alcohol — even at "moderate" levels, the liver works harder. GGT returning to normal after a few weeks of abstinence is a good diagnostic sign.

Coffee is one of the few foods with consistently favorable scientific evidence: regular coffee consumption (without added sugar) is associated with lower liver inflammation and a reduced risk of fibrosis.

Mediterranean diet — rich in vegetables, fruit, olive oil, fish and legumes — is associated with better liver enzyme profiles compared to a Western diet.

When to Seek Urgent Medical Attention

If elevated enzymes are accompanied by jaundice (yellowing of the skin or eyes), severe pain in the upper right abdomen, dark urine or intense nausea, these are signs that require prompt medical attention rather than a wait-and-see approach.
