What TSH Is and What It Tells You

TSH stands for thyroid-stimulating hormone, a chemical messenger produced by the pituitary gland, a small structure at the base of the brain. Its job is to signal the thyroid gland, located in the neck, to produce and release thyroid hormones called T4 and T3. These hormones influence how your body uses energy, regulates temperature, and supports heart, digestive, and brain function.

TSH works on a feedback loop. When thyroid hormone levels in the blood are low, the pituitary releases more TSH to encourage the thyroid to work harder. When thyroid hormone levels are high, the pituitary reduces TSH output. Because of this relationship, TSH is often more sensitive to early thyroid changes than T4 or T3 themselves, which is why it is typically the first test used to screen for thyroid function.

A single TSH value gives a snapshot of how the pituitary perceives thyroid activity at that moment. It does not diagnose a specific thyroid condition on its own, but it is a useful starting point for understanding whether the thyroid may be underactive, overactive, or functioning within an expected range.

Most laboratories report a standard adult TSH reference range of roughly 0.4 to 4.5 mIU/L, though some clinicians and functional medicine practitioners consider an "optimal" range closer to 0.5–2.5 mIU/L to reflect a lower likelihood of subtle thyroid dysfunction. Reference ranges can vary between labs due to differences in assay methods and calibration, so the range printed on your own lab report should always take priority over any general figures you see online.

Age, pregnancy, and certain health conditions can shift what is considered typical. For example, TSH reference ranges often differ during pregnancy, and some guidelines suggest slightly higher acceptable ranges in older adults. Why TSH May Be High

Why TSH May Be Low

A low TSH often indicates the pituitary senses higher-than-needed thyroid hormone levels, or in some cases, a pituitary-related issue. Possible reasons include:

  • Overactive thyroid (hyperthyroidism), such as Graves' disease
  • Excess thyroid hormone replacement medication
  • Early pregnancy, when TSH naturally dips
  • Non-thyroidal illness, especially during severe or prolonged sickness
  • Pituitary gland disorders affecting TSH production (less common)
  • Certain medications, including steroids or dopamine-affecting drugs

What to Do Next

If your TSH falls outside the expected range, a healthcare provider will often recommend retesting alongside free T4 and sometimes free T3 to get a fuller picture of thyroid function. Thyroid antibody tests, such as TPO antibodies, may also be considered if autoimmune thyroid disease is suspected.

Lifestyle factors such as sleep quality, stress, and certain supplements (particularly biotin, which can interfere with some TSH assays) are worth mentioning to your doctor before testing. It is also helpful to note any new medications, recent illnesses, or pregnancy status, as these can all influence results.
