What is it?
Amylase is an enzyme that breaks down starch into simpler sugars. In the body, amylase is produced by two main sources: the pancreas (pancreatic amylase) and the salivary glands (salivary amylase). Both forms enter the bloodstream and can be detected in a blood test.
An amylase test is most commonly ordered when pancreatic damage is suspected. However, this enzyme is not specific to the pancreas alone — its levels can change for other reasons as well. The test is performed using a venous blood sample, and the result is expressed in units per liter (U/L).
Pancreatic enzymes play a vital role in digestion. When pancreatic cells are damaged, amylase is released massively into the blood and its concentration rises sharply. This is why the test is one of the first ordered in cases of acute abdominal pain.
It is worth noting that a urine amylase test also exists. Since amylase is a relatively small protein, it is filtered through the kidneys and enters the urine. Urine amylase measurement is sometimes used as an additional diagnostic tool, especially when blood test results are unclear.
Reference range
The normal range of amylase in adults is typically 28–100 U/L. Specific reference limits may vary slightly depending on the laboratory and the methodology used.
It is important to know that a normal amylase level does not completely rule out pancreatic pathology. During chronic pancreatitis, the pancreas may be so severely damaged that it no longer produces enough enzyme — in that case, amylase remains normal or even low despite disease progression. This is called pancreatic enzyme depletion.
Reference ranges may differ slightly for children and elderly individuals. Some laboratories also separately measure pancreatic amylase (P-amylase), which is more specific to the pancreas — it accounts for about 40–45% of total blood amylase. Always compare your result with the reference interval provided by the laboratory.
Hypertriglyceridemia (very high triglyceride levels in the blood) can interfere with amylase measurement and lead to a falsely low result. If triglycerides exceed 5.6 mmol/L, it is worth informing your doctor.
Why might it be elevated?
Elevated amylase levels in the blood can be associated with several causes:
Acute pancreatitis. This is the most common cause of significant amylase elevation. Amylase can exceed the normal range by 3–5 times within hours of symptom onset. The most common causes of acute pancreatitis are gallstones and alcohol consumption.
Chronic pancreatitis. Amylase may be slightly elevated or normal, depending on the stage of the disease and the condition of pancreatic tissue.
Salivary gland inflammation (parotitis). Since the salivary glands also produce amylase, their inflammation can raise overall blood amylase levels. This is particularly relevant in mumps (epidemic parotitis).
Kidney dysfunction. Amylase is cleared through the kidneys. When kidney function is impaired, the enzyme accumulates in the blood even if the pancreas and salivary glands function normally.
Gallbladder and bile duct diseases. Gallstones blocking the pancreatic duct can cause amylase elevation and pancreatic inflammation.
Medications and alcohol. Some medications (opioids, diuretics) and regular alcohol consumption can affect amylase levels.
Other causes. Intestinal obstruction, ovarian cysts, ectopic pregnancy, and certain oncological diseases can rarely elevate amylase.
Why might it be decreased?
Decreased amylase levels in the blood are less common but can also be significant:
Chronic pancreatitis (late stages). Long-standing disease destroys the enzyme-producing cells of the pancreas, so amylase production decreases. This indicates that pancreatic tissue is significantly damaged.
Cystic fibrosis. This hereditary disease damages the pancreas and other organs, reducing enzyme production from childhood.
Severe liver damage. Liver cirrhosis or hepatitis can affect amylase levels through general metabolic disturbances.
Metabolic syndrome. Some studies suggest that decreased amylase may be associated with increased risk of metabolic syndrome and type 2 diabetes, although this relationship is still being studied.
Toxemia during pregnancy. In some cases, amylase may decrease due to liver and pancreatic changes during pregnancy complications.
What to do next?
If your amylase result deviates from the normal range, it is recommended to:
1. Discuss the result with your doctor — they will evaluate the context along with symptoms and other tests. Never interpret a single marker in isolation. 2. If the elevation is mild and there are no symptoms, your doctor may recommend repeating the test in 2–4 weeks to confirm or rule out the finding. 3. Additional tests may include lipase measurement (a more specific pancreatic marker), abdominal ultrasound, computed tomography, or other imaging studies. 4. Inform your doctor about medications you take, alcohol consumption habits, and any abdominal symptoms — pain, nausea, vomiting, or appetite changes.

Tracking over time
A single amylase measurement reflects only a momentary snapshot. Amylase levels can change rapidly — during acute pancreatitis, they rise within hours and return to normal within 3–5 days. Therefore, the timing of the test is very important for result interpretation.
If you have chronic pancreatic or digestive problems, periodic amylase monitoring helps your doctor assess disease progression. Together with lipase and other pancreatic function tests (elastase, fecal elastase), amylase provides valuable information about pancreatic health over time.
Regular recording of results allows you to notice trends that are more informative than individual numbers. Declining amylase in chronic pancreatitis may indicate progressive loss of pancreatic tissue.
