What Creatinine Is and What It Tells You

Creatinine is a waste product created when your muscles break down a compound called creatine, which is used for energy. Your body produces creatinine at a fairly steady rate, and it is filtered out of the blood by the kidneys and excreted in urine.

Because this process is so consistent, creatinine has become one of the most widely used markers of kidney function. When the kidneys are working well, creatinine is cleared efficiently and stays within a stable, predictable range. When kidney filtration slows down, creatinine tends to build up in the blood.

Doctors often look at creatinine alongside other markers, such as estimated glomerular filtration rate (eGFR) and blood urea nitrogen (BUN), to get a fuller picture of how well the kidneys are doing their job. On its own, creatinine gives a useful but partial snapshot.

Reference Range

A commonly used reference range for adult men is approximately 0.74 to 1.35 mg/dL, and for adult women approximately 0.59 to 1.04 mg/dL (or the equivalent in µmol/L for labs using SI units). The difference between sexes largely reflects average differences in muscle mass, since more muscle tissue generally means more creatinine production.

It is important to remember that reference ranges can vary somewhat between laboratories, depending on the testing method and population used to establish them. Always compare your result to the range printed on your own lab report, since that is the one most relevant to your specific test.

Age can also play a role, as muscle mass and kidney function naturally shift over the lifespan. Some labs provide separate pediatric or older-adult ranges as well.

Why It May Be High

Why It May Be Low

Lower-than-usual creatinine is less commonly discussed but can also occur for a number of reasons.

  • Lower muscle mass, which is common with aging, reduced physical activity, or certain chronic illnesses
  • Pregnancy, which increases blood volume and kidney filtration
  • A low-protein or vegetarian diet with reduced meat intake
  • Liver disease, which can reduce creatine production
  • Overhydration, which dilutes the blood

In most cases, a low creatinine level by itself is not a cause for concern, especially if other kidney markers look normal. It is generally interpreted in context rather than as a standalone finding.

What to Do Next

If your creatinine falls outside your lab's reference range, the most reasonable first step is usually a recheck, since single values can be affected by hydration, recent exercise, or lab variability. Repeating the test under similar, consistent conditions (such as similar hydration and timing) gives a clearer signal.

It is often helpful to look at creatinine together with eGFR, BUN, and urine tests such as albumin, since these paint a more complete picture of kidney health. Blood pressure and blood sugar levels are also relevant, given their close relationship with kidney function over time.

Simple lifestyle factors worth considering include staying adequately hydrated, moderating very high protein or meat intake before testing, and avoiding intense exercise the day before a blood draw. Any medications that might affect kidney function are worth discussing with your doctor, especially if a change in creatinine coincides with starting something new.

Tracking It Over Time

A single creatinine value offers limited information because it can shift with hydration, muscle mass, diet, and even the specific lab you use. What tends to be more meaningful is your personal trend across multiple tests.

A creatinine level that stays fairly stable over months or years, even near the edges of the reference range, is often less concerning than one that is steadily rising or falling. For people with known kidney risk factors, clinicians may recommend retesting every several months to a year, while others may only need it as part of routine annual bloodwork. Tracking your results over time, alongside eGFR, can help you and your doctor notice patterns early.
