What is D-Dimer
D-Dimer is a protein fragment that appears in the blood when the body breaks down blood clots. When a thrombus forms somewhere in the body, fibrinolysis — the clot breakdown process — simultaneously begins. D-Dimers are by-products of this breakdown that can be detected in a blood test.
This marker is especially useful as a "rule-out" test — if D-Dimer levels are normal, it is very unlikely that active thrombus formation is occurring. The test is usually performed from a venous blood sample.
Reference ranges
The upper reference limit for most laboratories is 0.5 µg/mL (or 500 µg/L FEU units). Below this limit, the result is considered negative. For older individuals (over 50 years), some laboratories apply an age-adjusted threshold: age × 10 µg/L. During pregnancy, D-Dimer levels naturally rise, so standard limits do not apply.
Why D-Dimer may be elevated
An elevated D-Dimer level indicates that clotting and fibrinolysis processes are occurring in the body. This may be associated with deep vein thrombosis, pulmonary embolism, disseminated intravascular coagulation (DIC), surgical interventions, or trauma. D-Dimer also rises during infections, inflammation, cancer, and liver failure. Pregnancy, advanced age, and prolonged bed rest can also increase this marker.
It is important to understand that an elevated D-Dimer level is not a diagnosis in itself — it is a signal that further investigation is needed.
Why D-Dimer may be low
A low or undetectable D-Dimer level is normal and indicates that there is no active blood clot formation or breakdown in the body. This is the best possible result when the test is performed due to suspected thrombotic event.
In very rare cases, a falsely low result may occur if the sample is taken at a very early stage of thrombosis or if symptoms began long ago (more than two weeks).
What to do next
If D-Dimer levels are elevated, the doctor usually orders additional tests: leg vein ultrasound (to rule out deep vein thrombosis), chest CT with contrast (for pulmonary embolism), complete blood count, and coagulation panel. If D-Dimer is normal and clinical probability is low, additional tests are usually not needed.
Do not interpret results on your own — always discuss them with your doctor, who will assess the full clinical picture.
Tracking over time
D-Dimer testing is not routinely performed in healthy individuals — it is ordered for specific symptoms or clinical suspicions. However, for patients taking anticoagulants or with a history of thrombosis, the doctor may periodically check this marker.
