What is NT-proBNP?

NT-proBNP (N-terminal pro-B-type natriuretic peptide) is a protein fragment released into the blood when the heart muscle (myocardium) experiences increased stress or load. When the walls of the heart chambers are stretched, due to increased blood volume or pressure, heart cells produce BNP (B-type natriuretic peptide). During this process, an inactive fragment called NT-proBNP is also released into the bloodstream, and this fragment is easier to measure in the laboratory than BNP itself.

It is important to understand that NT-proBNP reflects the functional workload of the heart, not its structure. It does not answer the question of what the heart looks like, but rather whether the heart is currently working under an additional burden.

This marker is measured in pg/mL and is especially important for assessing heart failure, a condition in which the heart cannot pump blood effectively.

Reference ranges

The primary threshold applied to adults under 75 years of age is 125 pg/mL. Values below this are generally considered normal. However, it is very important to note that NT-proBNP reference ranges depend strongly on age.

For older individuals (above 75 years), a higher threshold of up to 450 pg/mL is applied. This is expected, because the heart naturally experiences greater load with age.

Kidney function must also be taken into account. Impaired kidney function can raise NT-proBNP concentration in the blood even in the absence of heart problems.

Why it may be elevated

An elevated NT-proBNP indicates that the heart is experiencing greater than normal mechanical stress. There are several possible causes:

Heart failure is the most common cause of clinically significant NT-proBNP elevation. Dysfunction of the left or right heart chamber increases wall tension.

Arterial hypertension: sustained high blood pressure forces the left ventricle to work harder, causing the walls to thicken and stiffen.

Heart valve disease: narrowed or leaking valves create additional load on the heart.

Pulmonary embolism or elevated pulmonary vascular pressure affects the right ventricle.

Kidney insufficiency: reduced clearance of NT-proBNP by the kidneys can raise its concentration.

Age: older individuals have a naturally higher baseline NT-proBNP level.

Why it may be low

A low or normal NT-proBNP level is favorable. It indicates that the heart is not working under an extra burden, which is a positive sign.

Very low NT-proBNP values are not clinically problematic. In obese patients, relatively lower NT-proBNP values are sometimes observed, but this does not necessarily reflect better heart health. In such cases, the full clinical picture needs to be assessed more broadly.

What to do next

If NT-proBNP is elevated, it is important to discuss this with your doctor without delay. This marker is usually evaluated together with symptoms (shortness of breath, fatigue, leg swelling) and other investigations (ECG, echocardiogram).

An elevated NT-proBNP on its own is not a final answer. It is a signal that prompts further investigation. Your doctor will decide whether a cardiology workup is needed.

From a lifestyle perspective, factors important for heart health include: blood pressure control, reducing salt intake (especially in heart failure), monitoring fluid balance, and regular physical activity as approved by your doctor.

Monitoring over time

NT-proBNP is especially valuable as a dynamic marker. Changes over time can reflect treatment effectiveness or disease progression. For patients already being treated for heart failure, regular NT-proBNP monitoring helps the doctor assess whether therapy is working.

If the marker was measured as part of a routine check and is slightly elevated without any symptoms, the doctor may recommend repeating the test after a period of time or carrying out additional investigations.
