What Serum Iron Is and What It Tells You

Serum iron measures the amount of iron circulating in your blood, most of which is bound to a transport protein called transferrin. Iron is essential for making hemoglobin, the molecule in red blood cells that carries oxygen throughout the body, and it also plays a role in energy metabolism and immune function.

Unlike ferritin, which reflects your body's stored iron, serum iron reflects the iron actively moving through your bloodstream at the moment of the test. Because iron levels can shift quickly based on diet, time of day, and other factors, this test is usually interpreted alongside other markers rather than on its own.

Doctors often order serum iron together with total iron-binding capacity (TIBC), transferrin saturation, and ferritin to get a fuller picture of how your body is managing iron. Looking at these markers as a group gives more reliable information than serum iron alone.

Reference Range

Typical adult reference ranges are approximately 65–175 µg/dL for men and 50–170 µg/dL for women, though ranges can vary somewhat between laboratories depending on the testing method used. Always compare your result to the range printed on your own lab report, since that is the range calibrated to the specific assay used.

In SI units, serum iron is measured in µmol/L. Values can differ slightly by age as well, with some pediatric and older adult ranges varying from standard adult figures. If your result falls near the edge of the range, this alone is not necessarily meaningful and should be considered alongside your symptoms and other lab values.

Why It May Be High

A high serum iron result can have several possible explanations, including:

  • Recent intake of iron-rich foods or iron supplements before the blood draw

  • Liver conditions, since the liver plays a central role in iron metabolism

Because iron levels fluctuate throughout the day and are sensitive to recent meals, a single elevated result is often not concerning on its own and may be worth rechecking under standardized conditions, such as fasting in the morning.

Why It May Be Low

Lower-than-expected serum iron may be associated with:

  • Iron deficiency, from inadequate dietary intake or increased needs (such as during pregnancy)
  • Chronic blood loss, including from heavy menstrual periods or gastrointestinal sources
  • Malabsorption conditions, such as celiac disease, that reduce iron uptake from food
  • Chronic inflammation or infection, which can lower circulating iron even when stores are adequate
  • Chronic diseases affecting how the body regulates iron
  • Recent significant blood donation

Low serum iron does not automatically mean iron-deficiency anemia. Since inflammation alone can lower serum iron temporarily, additional markers like ferritin and inflammatory markers help clarify the underlying cause.

If your serum iron result is outside the expected range, it is generally worth discussing the finding with a doctor rather than trying to interpret it in isolation. A clinician may suggest repeating the test, ideally fasting and in the morning, since iron levels vary throughout the day.

Tracking your results over multiple tests gives a much clearer picture of your actual iron status and how it responds to diet, supplementation, or underlying health conditions.

Many people recheck serum iron every few months if they are actively addressing a deficiency or excess, or annually as part of routine health monitoring. Watching the trend alongside ferritin and CBC results over time tends to be more informative than focusing on any one value.
