What Is TIBC (Total Iron-Binding Capacity)

TIBC (Total Iron-Binding Capacity) is a laboratory marker that measures how much iron can be bound by proteins in blood plasma, primarily transferrin. Transferrin is the main iron-transport protein, carrying iron from the intestine and storage sites to cells that need it. When the body has little iron, the liver produces more transferrin — a biological signal saying "more iron needed". Because more transferrin means more available binding sites, TIBC rises. Conversely, when iron is excessive or the liver is damaged, TIBC falls.

TIBC is measured in µg/dL (some laboratories use µmol/L, with a conversion factor of 0.179):

  • General adult reference range: 250–450 µg/dL

These ranges do not differ significantly by sex, but TIBC naturally rises during pregnancy.

Why TIBC May Be Elevated

Elevated TIBC means there are many free transferrin binding sites in the blood. This is a classic iron-deficiency signal:

  • Iron-deficiency anemia: the most common cause.
  • Pregnancy: increasing iron requirements.
  • Long-term low dietary iron intake: vegetarians and vegans.
  • Bleeding: chronic blood loss.

Why TIBC May Be Low

Low TIBC means fewer free binding sites or reduced transferrin production:

  • Iron overload (hemochromatosis).
  • Anemia of chronic disease: inflammation suppresses transferrin production.
  • Liver disease: the liver produces transferrin.
  • Malnutrition or protein deficiency.
  • Chronic inflammation.

What to Do Next

A TIBC result is valuable only in context. Before drawing conclusions, it is worth looking at the full iron panel picture: serum iron, transferrin saturation percentage, and ferritin level. If TIBC is elevated and ferritin is low, this is a strong iron-deficiency signal. In that case it is useful to:

  • Review your diet and include iron-rich foods.
  • Vitamin C improves iron absorption, while drinking coffee or tea with meals can slow it.
  • Identify a possible source of chronic bleeding.

Monitoring Over Time

TIBC changes relatively slowly — it reflects processes over several weeks or months. This makes it a good long-term indicator of iron metabolism. If you are being treated for iron-deficiency anemia, monitoring TIBC every 3 months together with ferritin allows you to see whether the treatment is working.
