What is it?
Lipase is an enzyme that breaks down fats (triglycerides) into glycerol and fatty acids. The majority of lipase is produced by the pancreas, making this enzyme one of the most specific markers of pancreatic damage in the blood.
Pancreatic lipase is secreted into the duodenum, where it participates in fat digestion. Under normal conditions, a small amount of lipase enters the blood. However, when pancreatic tissue is damaged — due to inflammation, obstruction, or other causes — lipase is massively released into the blood and its concentration increases significantly.
Compared to amylase, lipase is a more specific indicator of the pancreas. It rises somewhat later than amylase but remains elevated longer — up to 8–14 days after an episode of acute pancreatitis. Therefore, lipase is a valuable diagnostic tool even if the patient presents several days after symptom onset.
Besides pancreatic lipase, other forms of lipase exist in the body: lipoprotein lipase (involved in fat metabolism in the blood), hepatic lipase, and gastric lipase. However, blood tests most commonly measure pancreatic lipase.
Reference range
The normal range of lipase in adults is typically 13–60 U/L. As with other enzymes, specific reference limits depend on the laboratory and methodology used.
A lipase elevation of more than 3 times the upper limit of normal (above 180 U/L) is one of the diagnostic criteria for acute pancreatitis according to international guidelines. This criterion, together with characteristic abdominal pain and imaging findings, forms the diagnostic triad of acute pancreatitis.
Smaller elevations (1–3 times above normal) may have various causes and do not necessarily indicate acute pancreatitis. Kidney dysfunction can lead to an elevated baseline lipase level, as the enzyme is partly cleared through the kidneys. Therefore, kidney function should be considered when evaluating the result.
In some patients, lipase may be persistently slightly elevated without clear pathology — this is called unexplained hyperlipasemia and typically does not require specific treatment.
Why might it be elevated?
Elevated lipase levels in the blood most often indicate pancreatic or adjacent organ pathology:
Acute pancreatitis. The primary cause of significant lipase elevation. Lipase typically rises within 4–8 hours of symptom onset and can exceed the normal range by 5–10 times. The most common causes are gallstones (about 40% of cases) and alcohol (about 30% of cases).
Chronic pancreatitis. Lipase may be slightly elevated during flare-ups, although between episodes it may remain normal or even decreased due to destruction of pancreatic tissue.
Cholecystitis (gallbladder inflammation). Gallbladder inflammation can cause moderate lipase elevation, especially if the inflammation affects adjacent pancreatic tissue.
Bile duct obstruction. Gallstones or other factors blocking the common bile or pancreatic duct can raise lipase due to increased pressure in the pancreatic duct.
Kidney failure. When lipase clearance through the kidneys is impaired, its blood level may rise. This is important to distinguish from true pancreatic pathology.
Intestinal obstruction. Bowel obstruction is sometimes associated with lipase elevation due to reflex pancreatic stimulation.
Certain medications. Some medications, including certain antibiotics and immunosuppressive agents, can affect lipase levels.
Why might it be decreased?
Decreased lipase levels in the blood rarely have clinical significance. Possible causes include:
Chronic pancreatic damage. Long-term destruction of pancreatic tissue (for example, in late stages of chronic pancreatitis) can reduce lipase production. This indicates that a large proportion of pancreatic acinar cells have been lost.
Cystic fibrosis. This genetic disease causes pancreatic insufficiency, reducing enzyme secretion. Cystic fibrosis is more commonly diagnosed in childhood.
Hypertriglyceridemia. Very high triglyceride levels in the blood can interfere with laboratory lipase measurement and produce falsely low results.
Other causes. Some rarer conditions associated with pancreatic atrophy or exocrine pancreatic insufficiency can lead to decreased lipase production.
In most cases, low lipase levels do not require separate investigation and are evaluated in the overall clinical context.
What to do next?
If your lipase result is abnormal:
1. Contact your doctor — elevated lipase may require urgent evaluation, especially if you experience abdominal pain, nausea, or vomiting. Severe abdominal pain combined with significantly elevated lipase is a reason to seek emergency care. 2. Your doctor may order additional tests: abdominal ultrasound, computed tomography, amylase measurement, liver function markers. 3. If the elevation is mild and there are no symptoms, repeating the test after several weeks and monitoring trends may be recommended. 4. Inform your doctor about alcohol consumption, medications, and any gastrointestinal symptoms — this will help identify the cause more precisely.

Tracking over time
Lipase trends over time can provide important information. During acute pancreatitis, lipase rises within hours and returns to normal within 1–2 weeks. If lipase remains elevated for longer than 2 weeks, this may indicate complications — a pseudocyst, abscess, or chronic process.
For individuals with risk factors (gallstones, regular alcohol consumption, family history, hypertriglyceridemia), periodic lipase monitoring can help detect pancreatic problems early.
Together with amylase, liver function markers, and imaging studies, lipase helps your doctor build a comprehensive picture of pancreatic health. Regular recording of results allows comparison of trends and faster response to changes.
