What is urine protein
Urine protein (PRO) is an indicator showing how much protein enters the urine. Healthy kidneys filter the blood and retain proteins in the body -- they should not enter the urine in significant amounts. Protein detected in urine (proteinuria) may signal damage to the kidney filter (glomeruli) or other conditions.
This test is most commonly performed as part of a routine urinalysis (OAM) using a rapid dipstick. It is simple, inexpensive, and informative as a primary screening test.
Reference ranges
Normal urine should contain very little or no protein. Dipstick results are usually reported as: negative, trace, or positive with gradation (1+, 2+, 3+, 4+). Quantitative tests measure mg/dL or g/L.
It is important to know that a single positive result does not necessarily mean disease -- protein in urine can appear temporarily due to various causes.
Why protein may appear in urine
Temporary (usually harmless) causes: intense physical exercise, fever, dehydration, stress, and prolonged standing (orthostatic proteinuria -- especially common in young people). These causes usually produce a small, transient protein elevation.
More persistent causes may be related to kidney diseases: glomerulonephritis, diabetic nephropathy, hypertensive nephropathy, or other forms of kidney damage. Urinary tract infection can also cause protein to appear in urine, but usually other changes are also detected (leukocytes, nitrites).
Why protein may be absent in urine
A negative urine protein result means the kidney filter is functioning properly and proteins remain in the blood, where they belong. This is a normal, expected result.
In rare cases, the dipstick may not detect certain protein types (e.g., light chains), so if there is specific suspicion, the doctor may order more precise tests.
What to do next
If protein is found in urine, the doctor may first repeat the test after a few days or weeks, as the cause is often temporary. If protein persists, more precise evaluation may be needed: urine albumin-to-creatinine ratio (ACR), 24-hour urine collection, or blood creatinine and eGFR testing.
It is important to control blood pressure and blood sugar levels, as hypertension and diabetes are the most common causes of chronic proteinuria. This is a topic for discussion with your doctor.
Monitoring over time
