What are basophils (BASO%)
Basophils are the smallest group of white blood cells (leukocytes). They make up less than 2% of all leukocytes and are part of the innate immune system. Basophils release histamine, which triggers allergic reactions — nasal congestion, itching, swelling. They also participate in inflammatory processes and help regulate the immune response. BASO% shows the share of basophils among all leukocytes.
This marker is measured as part of a complete blood count with differential.
Reference ranges
In adults, the basophil percentage is normally between 0 and 2% of all leukocytes. Most healthy individuals have a result of 0–1%. Because there are very few basophils in the blood, even a change of a single cell can shift the percentage result. For this reason, it is important to interpret this marker together with the absolute count (BASO#).
Why basophils may be elevated
Basophilia (elevated basophil count) is rare but can be associated with several conditions. Allergic diseases — asthma, urticaria, atopic dermatitis — are sometimes accompanied by mild basophilia. Chronic inflammatory diseases such as ulcerative colitis or rheumatoid arthritis may also have an effect.
Reduced thyroid function (hypothyroidism) is one of the known reasons why basophils may be slightly elevated. Iron deficiency anaemia is occasionally associated with basophilia. Myeloproliferative diseases (e.g., chronic myeloid leukaemia) can cause significant and persistent basophil elevation — in such cases other blood markers are usually also altered.
Why basophils may be decreased
A decreased basophil percentage is usually not clinically significant, since the normal count is already close to zero. Use of glucocorticoids (e.g., prednisolone), acute infections, and intense physical or emotional stress can reduce basophil levels further. During pregnancy, the basophil fraction may also decrease.
If other blood markers are within normal limits, a low BASO% is generally not a cause for concern.
What to do with your result
A single BASO% deviation is enough for a doctor to take note, but not enough to diagnose a disease. The doctor will assess the entire white cell differential — neutrophils, lymphocytes, monocytes, and eosinophils — together with clinical symptoms. If basophilia recurs, further tests may be ordered: thyroid hormones, iron markers, or a haematology referral.
A balanced diet, physical activity, and adequate rest support immune health.
Monitoring over time
Because the basophil count is very low, fluctuations between tests are normal and do not always indicate a true change. A trend across several tests is more informative than a single result. If you have a known allergic condition or thyroid problem, your doctor may recommend monitoring this marker as well.
