What is Hematocrit (HCT)
Hematocrit (HCT) is the percentage of red blood cells (erythrocytes) in the total blood volume. In other words, this marker shows how much of your blood is made up of cells and how much is plasma (the liquid portion).
Hematocrit is closely related to hemoglobin and red blood cell count. If red blood cells carry oxygen, hematocrit helps assess whether the blood is thick enough to perform this function.

Why Hematocrit May Be Elevated
Elevated hematocrit means red blood cells make up a larger share of the blood than usual. This may be due to:
- Dehydration — the most common cause. As fluid levels drop, blood becomes thicker
- Smoking — carbon monoxide stimulates the body to produce more red blood cells
- Living at high altitude
- Chronic lung diseases (COPD, asthma)
- Polycythemia vera — a rare bone marrow disorder
- Intense exercise — a temporary increase
High hematocrit increases blood viscosity, which may be associated with greater cardiovascular strain.
Why Hematocrit May Be Low
Low hematocrit is most often associated with anemia or increased blood plasma volume:
- Iron deficiency — red blood cells are smaller and contain less hemoglobin
- Vitamin B12 or folic acid deficiency
- Chronic diseases — kidney failure, inflammation
- Blood loss — both obvious and slow internal bleeding
- Pregnancy — plasma volume physiologically increases, causing hematocrit to fall
- Certain infectious diseases
What to Do With Your Results
Hematocrit is evaluated alongside other complete blood count markers:
- If low — it is important to check hemoglobin, red blood cell count, ferritin, and iron
- If high — assess whether dehydration is present and check red blood cell count
- A minor deviation, especially during menstruation or after intense exercise, does not necessarily indicate disease
If the deviation recurs across several consecutive tests, it is worth discussing with your doctor.
Tracking Over Time
Hematocrit can fluctuate due to dehydration, physical activity, and even the time of year. A single measurement does not always reflect the true picture. By tracking hematocrit over time, you can see whether it is a temporary fluctuation or a developing trend.
