What are High Fluorescence Reticulocytes (HFR%)?

HFR% represents the highest-fluorescence fraction: the youngest, most RNA-rich reticulocytes in circulation. These cells were released from the bone marrow within the past day or so, making HFR% a near real-time window into marrow activity.

Because this fraction is so short-lived, it responds to changes in marrow activity faster than any other reticulocyte parameter — or indeed faster than hemoglobin or red cell indices.

Why Might It Be Elevated?

An elevated HFR% means the bone marrow is releasing very immature reticulocytes — a sign of intense erythropoietic activity:

  • Acute blood loss (response within 24–48 hours).
  • Hemolytic anemia — compensatory burst of marrow activity.
  • Treatment response — first reticulocyte fraction to rise after iron/B12/folate therapy.
  • Erythropoietin therapy.

Why Might It Be Low?

A low HFR% combined with anemia suggests the marrow is not keeping up with demand:

  • Bone marrow failure or suppression (aplastic anemia, chemotherapy).
  • Nutritional deficiency constraining marrow output.
  • Anemia of chronic disease.
  • Renal insufficiency (reduced erythropoietin).

What to Do Next

HFR% should always be interpreted as part of the complete blood count and clinical picture. Your doctor will consider it alongside total reticulocyte count, hemoglobin trend, MFR%, and LFR%.

Monitoring Over Time

HFR% is the fastest-responding marker in reticulocyte analysis — changes can be detected within 1–3 days. After chemotherapy, a recovering HFR% signals marrow regeneration. Because of its rapid kinetics, frequent serial measurements are more informative than a single snapshot.
