What Cortisol (AM) is and what it tells you

Cortisol is a hormone produced by the adrenal glands, small glands located above the kidneys. It plays a role in regulating blood sugar, blood pressure, metabolism, inflammation, and the body's response to stress. Cortisol production is controlled by a signaling pathway involving the brain (hypothalamus and pituitary gland) and the adrenal glands, often called the HPA axis.

Cortisol naturally follows a daily rhythm, rising sharply in the early morning hours and gradually declining throughout the day. This is why a morning (AM) blood draw, typically between 7 and 9 a.m., is the standard way to test cortisol. Why it may be low

Lower than expected morning cortisol may be associated with:

  • Recent or long-term use of corticosteroid medications, which can suppress the body's own cortisol production even after stopping the medication

  • Certain lab or assay variability

As with high values, a single low reading warrants context rather than immediate concern, especially if the sample timing or recent health circumstances could explain it.

What to do next

If your AM cortisol result falls outside the expected range, it is generally reasonable to discuss a recheck with your doctor, ideally with attention to consistent morning timing. Depending on the pattern of results and any symptoms present, a clinician may also consider looking at related markers, such as ACTH, DHEA-S, or electrolytes like sodium and potassium, which can provide additional context about adrenal function.

Lifestyle factors that influence the stress-response system, such as sleep quality, regular exercise, and stress management, may play a supporting role in overall hormonal balance, though they are not a substitute for medical evaluation when levels are notably abnormal.

It is worth mentioning to your doctor any medications you take, including inhaled or topical steroids, as well as recent illness, unusual stress, or sleep disruption around the time of testing, since these can all influence the result.

Because cortisol is sensitive to timing, stress, sleep, and daily variability, a single measurement offers limited information on its own. Tracking morning cortisol over several tests, taken at a consistent time of day, can help distinguish a temporary fluctuation from a more persistent pattern.

Many people retest cortisol only when clinically indicated, such as when following up on an abnormal result or monitoring a known adrenal or pituitary condition. Keeping a personal record of your results alongside notes on sleep, stress, and medication use can make trends easier to interpret over time.
