What Prolactin Is and What It Tells You

Prolactin, often abbreviated PRL, is a hormone made by the pituitary gland, a small structure at the base of the brain. It is best known for its role in milk production after childbirth, but it is present in everyone's blood at low levels regardless of sex or reproductive status.

Beyond lactation, prolactin interacts with the reproductive system and can influence menstrual cycles, libido, and fertility when levels move outside the usual range. It is also sensitive to stress, sleep, and even the act of having blood drawn, which is why interpretation usually looks at the whole clinical picture rather than a single number.

A prolactin test is commonly ordered when someone reports irregular periods, unexplained milk production (in men or women who are not breastfeeding), fertility concerns, or symptoms that suggest a pituitary issue, such as persistent headaches or vision changes.

Reference Range

Reference ranges for prolactin vary by laboratory, by the assay method used, and sometimes by sex. As a general guide, many adult labs report approximately 4–15.2 ng/mL for men and 4.8–23.3 ng/mL for women, with units usually given in ng/mL (sometimes converted to mIU/L by some labs).

These figures are typical adult ranges only. Because testing methods differ between laboratories, the reference range printed on your own lab report is the one that should guide interpretation of your result, not a number found online. Pregnancy substantially raises prolactin as a normal physiological change, and postmenopausal ranges can differ slightly as well.

Why Prolactin May Be High

Elevated prolactin, sometimes called hyperprolactinemia, has several possible explanations, ranging from harmless to ones worth medical attention.

  • Pregnancy and breastfeeding, which naturally raise prolactin
  • Stress, including the stress of blood draws, intense exercise, or poor sleep the night before testing
  • Certain medications, including some antipsychotics, antidepressants, and medications for nausea or high blood pressure
  • Nipple stimulation or chest wall irritation shortly before the test
  • An underactive thyroid (hypothyroidism), which may be associated with higher prolactin
  • A benign pituitary growth called a prolactinoma, one of the more common reasons for a clinically significant elevation
  • Kidney disease, which can slow prolactin clearance from the body
  • Non-fasting samples or the timing of the blood draw, since prolactin can fluctuate through the day

A single mildly elevated result is often repeated before further evaluation, since transient spikes are common.

Why Prolactin May Be Low

Low prolactin is less frequently discussed and, in most healthy adults, is not a marker doctors focus on the way they do with high results.

  • Certain medications, including dopamine agonists sometimes used for other pituitary conditions
  • Pituitary underactivity (hypopituitarism), where multiple pituitary hormones may be reduced
  • Lab or assay variability, since very low readings can sometimes reflect measurement quirks rather than a true physiological issue

Because isolated low prolactin rarely causes symptoms on its own, it is usually interpreted alongside other pituitary hormones rather than in isolation.

What to Do Next

If your prolactin result falls outside your lab's reference range, the most useful first step is usually a repeat test, ideally fasting, mid-morning, and after avoiding strenuous exercise, sexual activity, or nipple stimulation beforehand, since these can transiently raise levels.

It can help to look at related markers together with prolactin, including:

  • Thyroid-stimulating hormone (TSH) and thyroid hormones, since thyroid function closely interacts with prolactin
  • Estrogen, testosterone, or other reproductive hormones, depending on symptoms
  • Kidney function markers, if relevant to your history

Lifestyle factors such as consistent sleep, stress management, and reviewing current medications with a clinician can also be worth discussing, since several everyday factors influence prolactin levels. Any persistent, unexplained elevation, especially alongside symptoms like vision changes, headaches, or unexpected milk production, is worth bringing to a doctor's attention for further evaluation, which may include pituitary imaging.

Tracking It Over Time

Because prolactin can shift with stress, sleep, medication timing, and even the specific moment of the blood draw, one result offers limited information on its own. Tracking prolactin over several tests, taken under similar conditions, gives a much clearer sense of whether a level is stable, trending up, or trending down.

Many people recheck prolactin every few months if an initial result was mildly abnormal, or as advised by a clinician if monitoring a known pituitary condition or medication effect. Keeping notes on timing, stress levels, and medications alongside each result can make trends easier to interpret.
