What Is Zinc (Zn)

Zinc is an essential trace element involved in hundreds of enzymatic reactions in the human body. Its effects are wide-ranging: from supporting the immune system and wound healing to DNA synthesis, protein production, and normal taste and smell perception.

A serum zinc test measures the amount of zinc in blood plasma or serum. It is important to understand that only about 1% of the body's total zinc circulates in the blood — the rest is inside cells and tissues. Because of this, serum zinc is not an ideal marker when the deficiency is mild, but it remains the most widely used clinical method for assessing zinc status. When having the test, note whether the blood was drawn fasting, as zinc levels in the blood can temporarily decrease after eating.

Reference Ranges

Zinc is measured in µg/dL (some laboratories use µmol/L, where the conversion factor is 0.153):

  • Normal for adults: 70–120 µg/dL

Ranges do not differ significantly by sex, although zinc levels often naturally decrease during pregnancy due to increased demand and fluid volume expansion. In older adults, zinc absorption from food may decline, making the lower end of the range particularly relevant for this group.

Why Zinc (Zn) May Be Elevated

Elevated serum zinc is a rare occurrence from natural sources — it is usually related to excess external zinc intake:

  • Zinc supplement overdose: excessively high supplement doses are the most common cause of elevated zinc.
  • Use of zinc-containing products: certain ointments, mouthwashes, or antacid preparations can cause absorption.
  • Industrial exposure: workers in certain occupations may be exposed to zinc dust or fumes.
  • Acute infectious process: the body may temporarily redistribute zinc into the blood as an acute-phase response, though this is more of a transient fluctuation.

Excessively high zinc levels can interfere with copper absorption and cause copper deficiency, so long-term excess requires attention.

Why Zinc (Zn) May Be Low

Zinc deficiency is far more common than excess and can affect many body systems:

  • Inadequate dietary intake: zinc is abundant in red meat, seafood (especially oysters), and legumes. A strict vegetarian or vegan diet without compensation can cause deficiency, as plant-based zinc is less well absorbed due to phytic acids.
  • Malabsorption: Crohn's disease, celiac disease, or other digestive tract disorders reduce zinc absorption.
  • Chronic diseases: diabetes, liver disease, and kidney failure are often associated with lower zinc levels.
  • Alcoholism: alcohol increases zinc excretion in urine and reduces its absorption.
  • Pregnancy and breastfeeding: increased demands can quickly deplete stores.
  • Intense sweating activity: zinc is lost through sweat during exercise.

What to Do Next

If serum zinc is low, the first step is to review your diet. The best dietary sources of zinc are: seafood, red meat, poultry, legumes, and nuts.

Plant-based zinc absorption can be improved by soaking legumes in water before cooking — this reduces phytic acid content. Vitamin C may also slightly improve zinc absorption.

Before starting zinc supplements, consult your doctor — excessive doses can disrupt copper and iron metabolism.

Monitoring Over Time

Serum zinc can fluctuate depending on recently consumed food and the body's inflammatory state (during the acute phase, zinc is transferred from the blood to tissues, so it may appear lower than it actually is). Therefore, a single measurement is interpreted cautiously.

If you change your diet or start supplements, repeat the test after 2–3 months to assess whether there has been a real change. Also monitor copper levels, especially if zinc supplementation is long-term — this will help prevent copper deficiency.
