What are Lymphocytes (LYMPH%)

Lymphocytes are a group of white blood cells that form the foundation of the adaptive immune system. Unlike neutrophils, which respond quickly and non-specifically, lymphocytes are specialised: they recognise specific pathogens, form immune memory, and coordinate long-term immune responses.

There are three main types of lymphocytes: T cells (involved in the cellular immune response, destroying virus-infected cells), B cells (produce antibodies), and NK (natural killer) cells (destroy tumour cells and virus-damaged cells without prior antigen recognition). LYMPH% shows the percentage of all lymphocytes among all leukocytes.

Reference ranges

Lymphocyte percentage (LYMPH%) in adults:

  • Normal: 20–45 %

These ranges are general for adults and do not differ significantly by sex. In children, the lymphocyte percentage is naturally higher — this is a normal physiological feature that changes during maturation. In older adults, the lymphocyte count may decrease slightly, reflecting the natural process of immune ageing (immunosenescence).

As with neutrophils, the absolute lymphocyte count (ALC, cells per µL) and the percentage together provide a fuller picture — they should be interpreted in context.

Why Lymphocytes (LYMPH%) may be elevated

An increase in the lymphocyte percentage is called lymphocytosis. The most common causes:

  • Viral infections: this is the most common cause. Infectious mononucleosis (caused by Epstein-Barr virus), cytomegalovirus (CMV), influenza, coronaviruses, and many other viral infections cause reactive lymphocytosis.
  • Pertussis (whooping cough): a bacterial infection that paradoxically causes marked lymphocytosis.
  • Chronic lymphocytic leukaemia (CLL): a serious condition of clonal lymphocyte proliferation, presenting with persistent and often very high lymphocyte elevation.
  • Autoimmune diseases: in some autoimmune conditions, lymphocyte activation may increase their percentage.
  • After intense physical activity: short-term lymphocytosis may occur after very intense exercise.

Why Lymphocytes (LYMPH%) may be low

A decrease in the lymphocyte percentage may result from a true decrease in lymphocytes (lymphopenia) or from relative changes — an increase in other cell types:

  • Bacterial infections and sepsis: when neutrophils increase significantly, the lymphocyte percentage automatically decreases.
  • Corticosteroids and immunosuppressants: these drugs reduce lymphocyte counts in the blood by redistributing them to tissues or suppressing production.
  • HIV infection: HIV specifically destroys CD4+ T lymphocytes, causing their number and percentage to decline.
  • Radiation therapy and chemotherapy: bone marrow suppression reduces production of all leukocytes, including lymphocytes.
  • Chronic stress: prolonged exposure to stress hormones (cortisol) is associated with lower lymphocyte counts.
  • Poor nutrition and zinc deficiency: nutrients are important for normal lymphocyte maturation.

What to do next

A lymphocyte percentage deviation is most often reactive — meaning it reflects some process in the body rather than being a problem itself. First, consider the broader context: are there symptoms of infection, are medications being taken, or was there recent intense physical exertion.

If the increase is related to a viral infection, normalisation can be expected within 2–6 weeks. If the deviation is persistent and there is no clear cause, the doctor may recommend further testing, including lymphocyte subpopulation analysis (determining T, B, NK cell ratios).

Monitoring over time

The lymphocyte percentage can fluctuate quite dynamically, especially during infections. For this reason, a single measurement often requires repetition — especially if the deviation is significant or unexpected.

In the long term, LYMPH% monitoring is informative for people with chronic immune system diseases, those on immunosuppressive therapy, or after oncological treatment. An annual blood count provides the opportunity to track trends and detect significant changes in time.
