What it is
MCHC (mean corpuscular hemoglobin concentration) is a blood test parameter that shows the average concentration of hemoglobin in a single red blood cell. Simply put, this parameter indicates how densely hemoglobin is “packed” inside a red blood cell. Hemoglobin is a protein that binds oxygen in the lungs and carries it to the tissues, so its concentration in red blood cells is directly related to efficient oxygen transport in the body.
MCHC is one of the red blood cell indices calculated during a complete blood count (CBC). This parameter helps the doctor assess whether red blood cells are adequately filled with hemoglobin and can help determine the nature of certain anemias.
Reference ranges
The reference range for MCHC in adults is typically 32–36 g/dL. These limits may vary slightly depending on the laboratory, so it is always worth comparing your result with the specific laboratory’s stated reference ranges. A value below 32 g/dL is considered low, and a value above 36 g/dL is considered high.
Why it may be high
An elevated MCHC is less common than a decreased one and may be associated with several causes. One of them is spherocytosis, a hereditary condition in which red blood cells take on a spherical shape and become smaller, causing the hemoglobin concentration within them to be relatively increased. Autoimmune hemolytic anemia, in which the body’s immune system destroys its own red blood cells, can also lead to an elevated MCHC.
Dehydration is a common physiological cause — when the volume of fluid in the blood decreases, red blood cells may “shrink,” and the hemoglobin concentration within them relatively increases. Additionally, laboratory errors, such as sample lipemia or hemolysis in the test tube, can sometimes falsely elevate the MCHC value.
Why it may be low
A decreased MCHC most commonly indicates that red blood cells are insufficiently saturated with hemoglobin — they are called hypochromic. The most common cause is iron deficiency anemia, when the body lacks iron to produce hemoglobin. This can occur due to inadequate dietary iron intake, chronic bleeding (for example, from the gastrointestinal tract or due to heavy menstruation), or impaired iron absorption.
Thalassemia — a hereditary disorder of hemoglobin production — can also lead to a low MCHC. Chronic diseases, such as chronic infectious or inflammatory conditions, can disrupt iron metabolism in the body and reduce hemoglobin concentration in red blood cells. Lead poisoning, although rare, is another possible cause.
What to do next
If your MCHC value is outside the reference range, it is important not to panic — a single parameter alone does not allow a diagnosis to be made. First, consult your doctor, who will evaluate MCHC together with other blood test parameters such as MCV, MCH, hemoglobin, and hematocrit.
Your doctor may recommend additional tests, for example, iron metabolism markers (ferritin, serum iron, transferrin), vitamin B12 and folic acid levels, or a peripheral blood smear review. Depending on the results, dietary adjustments or other actions may be discussed.
Maintain a balanced diet and consume enough iron-rich foods — red meat, legumes, dark green leafy vegetables. Drink enough fluids to avoid dehydration.
Tracking over time
Monitoring MCHC values across several tests over time is more valuable than a single test result. Repeat blood tests allow you to spot trends — whether the parameter is changing or remaining stable. This is especially important if your doctor is applying a specific treatment strategy, such as prescribing iron supplements.
