What are Monocytes (MONO%)
Monocytes are the largest white blood cells and part of the innate immune system. They circulate in the blood and, upon entering tissues, transform into macrophages or dendritic cells — specialised cleaning and signalling elements. Monocytes recognise and engulf foreign bodies, damaged cells, and infection remnants. MONO% shows the proportion of monocytes among all white blood cells.
This marker is measured as part of a complete blood count with differential. The result is expressed as a percentage of total leukocytes.
Reference ranges
The monocyte percentage in adults usually ranges from 2 to 10 % of all white blood cells. Some laboratories report a narrower interval, for example 2–8 %. In children, normal values may differ slightly depending on age. Always compare your result with the specific laboratory's reference range, as methodologies and reagents may vary.
Why monocytes may be elevated
An elevated monocyte percentage (monocytosis) can be associated with several causes. Chronic infections — tuberculosis, endocarditis, certain fungal diseases — often stimulate monocyte production. Autoimmune diseases such as systemic lupus erythematosus or rheumatoid arthritis may also raise monocyte levels. After major surgery or trauma, the body temporarily increases monocyte production for tissue repair.
Certain haematological conditions, including chronic myelomonocytic leukaemia, can present with persistently elevated monocyte levels. Long-term stress and chronic inflammation (e.g., obesity) are also associated with a higher monocyte proportion in the blood.
Why monocytes may be low
A decreased monocyte percentage (monocytopenia) occurs less frequently. It may result from medications that suppress bone marrow function, such as chemotherapy or immunosuppressive drugs. Some bone marrow disorders can reduce monocyte production. Acute infections sometimes temporarily decrease circulating monocyte levels, as they migrate into tissues to fight infection.
Very low monocyte levels may indicate a weakened immune response, and it is worth consulting your doctor in such cases.
What to do with your result
A single abnormal result does not always mean disease. The doctor evaluates the monocyte percentage together with the absolute count, other elements of the leukocyte differential, and clinical symptoms. If the monocyte count is consistently rising across several tests, the doctor may order additional investigations — inflammatory markers (CRP), iron metabolism, or specific infection tests.
A healthy lifestyle — balanced nutrition, regular physical activity, adequate sleep, and stress management — helps maintain normal immune function.
Monitoring over time
A single monocyte percentage provides limited information. Tracking the trend across several tests reveals whether monocytes are consistently rising, falling, or fluctuating. This is especially valuable after treatment, infections, or during long-term illnesses. Regular blood tests (e.g., every 3–6 months, if the doctor recommends) help see the overall picture.
