What are Neutrophils (NEUT%)

Neutrophils are the most abundant white blood cells (leukocytes) and the first line of immune defence against bacterial infections and fungal diseases. They are phagocytes — specialised cells that recognise, engulf, and destroy harmful micro-organisms.

NEUT% shows the percentage of neutrophils among all leukocytes. This marker is part of the standard blood count with differential (CBC with differential). It is important to understand the difference: NEUT% (percentage) and the absolute neutrophil count (ANC, cells per µL) provide complementary information. The percentage shows relative distribution, while the absolute count shows the actual quantity. Both markers are often evaluated together.

Neutrophil percentage (NEUT%) in adults:

  • Normal: 40–70 %

These ranges are general for adults and do not differ significantly by sex. In children, ranges vary by age — in newborns, neutrophils make up a larger proportion, which later decreases to adult norms. Remember that the percentage depends on the distribution of all leukocytes — if one cell type increases significantly, the other's percentage automatically decreases, even though the absolute count may remain unchanged.

Why Neutrophils (NEUT%) may be elevated

An increase in neutrophil percentage is called neutrophilia. The most common causes:

  • Bacterial infections: the classic signal. Bacterial pneumonia, urinary tract infections, skin infections, or sepsis cause rapid neutrophil increases.
  • Acute inflammation or tissue damage: myocardial infarction, trauma, surgery, or burns can cause reactive neutrophilia.
  • Corticosteroids: these drugs stimulate neutrophil release from bone marrow and may raise the percentage even without infection.
  • Physical stress: intense exercise, emotional stress, or smoking can temporarily increase the neutrophil percentage.
  • Chronic inflammation: some autoimmune or metabolic diseases are associated with persistent mild neutrophil elevation.
  • Myeloproliferative diseases: rare bone marrow disorders that can cause pathological neutrophilia.

Why Neutrophils (NEUT%) may be low

A decrease in neutrophil percentage (neutropenia when the absolute count drops, or relative neutropenia in percentage terms) can occur in several scenarios:

  • Viral infections: viruses often cause lymphocyte increases, which relatively reduce the neutrophil percentage. Influenza, EBV, COVID-19 are typical examples.
  • Medications: chemotherapy, certain antibiotics, antithyroid drugs, and other medications can suppress neutrophil production.
  • Bone marrow disorders: aplastic anaemia or other bone marrow diseases impair blood cell production.
  • Autoimmune neutropenia: the immune system mistakenly attacks neutrophils.
  • B12 or folate deficiency: these vitamins are needed for normal leukocyte production.

What to do next

NEUT% is evaluated together with the full blood count profile — lymphocytes, monocytes, absolute neutrophil count, and total leukocyte count. A single marker fluctuation without context often does not explain the full picture.

If the increase is related to acute infection or stress, neutrophils usually normalise after the underlying process resolves. A temporary small deviation often reflects a normal immune response.

Monitoring over time

Each time a blood count is performed, neutrophil trends can be observed. In acute infections, normalisation can be monitored within 1–2 weeks after recovery.

A chronically elevated or decreased NEUT% without clear cause requires closer monitoring and further investigation. During routine check-ups, the blood count is valuable — it provides a snapshot of the immune system and reveals trends not visible from a single result.
