What are Nucleated Red Blood Cells (NRBC#)
Nucleated red blood cells (NRBC) are immature red blood cells that still retain their cell nucleus. Normally, red blood cells lose their nucleus in the bone marrow before entering the bloodstream, so NRBCs are usually undetectable in adult blood. Their presence in peripheral blood indicates that the bone marrow is working unusually intensely or is damaged. NRBC# shows the absolute count of nucleated red blood cells.
This marker is measured by automated haematology analysers and is reported in thousands of cells per microlitre (10³/µL).
Reference ranges
In adult blood, nucleated red blood cells should be absent — the normal value is 0 (undetectable). In newborn blood, NRBCs are a normal finding in the first days of life and usually disappear within a week.
Why nucleated red blood cells may appear in blood
The presence of NRBCs in adult blood always requires medical attention. The most common causes:
Severe anaemia — especially haemolytic (when red blood cells are rapidly destroyed) or megaloblastic anaemia. The body, trying to compensate for red blood cell deficiency, releases immature forms into the blood prematurely.
Severe infections and sepsis can trigger NRBC appearance, particularly in intensive care patients. Research shows that the presence of NRBCs in critically ill patients is associated with a worse prognosis.
Bone marrow damage — myelofibrosis, bone marrow metastases, myelodysplastic syndrome — can lead to immature cell release into the bloodstream. Acute hypoxia (oxygen deficiency), severe trauma, and burn conditions can also temporarily cause NRBCs.
Why nucleated red blood cells may be low
Since the normal value is zero, a decrease is not evaluated. An undetectable NRBC count is normal.
What to do with your result
If NRBCs are detected in your blood, the doctor will take this seriously. Additional tests are usually ordered — reticulocytes, haemoglobin, haptoglobin, LDH, bilirubin, iron markers — to determine the cause. If bone marrow damage is suspected, a bone marrow biopsy may be recommended.
It is important to understand that the presence of NRBCs is a signal, not a diagnosis — the underlying cause can vary widely.
Monitoring over time
NRBC monitoring is relevant when treating severe anaemia or haematological diseases. Successful treatment should lead to the disappearance of NRBCs from peripheral blood. If NRBCs were detected once and subsequently disappeared, this may indicate temporary bodily stress.
