What are Nucleated Red Blood Cells (NRBC%)

Nucleated red blood cells (NRBC) are immature red blood cells that still retain the cell nucleus. In healthy adults they are normally absent from the blood — they complete maturation in the bone marrow and enter the circulation without a nucleus. NRBC% expresses the number of nucleated red blood cells per one hundred white blood cells.

This parameter is detected by automated hematology analyzers as part of a complete blood count.

Reference Range

The normal value for adults is 0% — nucleated red blood cells should not be detectable in peripheral blood. In newborns they may be present during the first days of life, which is physiological and resolves on its own.

Why might NRBC% be elevated?

The appearance of nucleated red blood cells in adult blood is always a clinically significant signal. Main causes:

Intensive erythropoiesis — when the body urgently produces new red blood cells, e.g. in severe hemolytic anemia (breakdown of red blood cells), acute blood loss, or severe iron deficiency. The bone marrow rushes and releases immature cells.

Bone marrow infiltration — malignant processes (leukemia, lymphoma, carcinoma with bone marrow metastases) or myelofibrosis can disrupt normal hematopoiesis and allow immature cells to enter the bloodstream. This is called a leukoerythroblastic reaction.

Severe systemic conditions — sepsis, shock, hypoxia, severe burns — can cause NRBC appearance in critically ill patients. Studies show that the presence of NRBCs in such circumstances is an unfavorable prognostic marker.

Why might NRBC% be decreased?

Decrease is not an applicable concept since the normal value is zero. An undetectable NRBC% is a normal, healthy result.

What to do next?

If NRBCs were detected in your test, the doctor will evaluate this in context — considering hemoglobin level, reticulocytes, white blood cell differential, and clinical symptoms. Additional tests may include checking hemolysis markers (LDH, haptoglobin, bilirubin), iron metabolism parameters, and if necessary, bone marrow examination.

It is important to know that NRBC detection is a signal prompting deeper diagnostic workup — it does not by itself indicate a specific disease.

Monitoring over time

NRBC% monitoring is relevant during treatment of hematological diseases or severe anemias. Successful treatment should lead to NRBC% returning to zero. If NRBCs were detected only once and disappeared in a subsequent test, it may have been a transient response of the body to stress or infection.
