What are Red Blood Cells (RBC)

Red blood cells (RBC) are the most abundant cells in the blood. Their primary function is to transport oxygen from the lungs to the tissues and return carbon dioxide back. Each red blood cell contains hemoglobin — a protein that binds oxygen and gives blood its red color.

Red blood cells are produced in the bone marrow, and their lifespan is about 120 days. They are then broken down in the spleen and liver, while the body continuously produces new ones. The RBC count in the blood indicates whether the body is receiving enough oxygen.

The RBC count is measured in millions per microliter of blood (10⁶/µL or 10¹²/L).

  • Men: 4.5–5.9 million/µL
  • Women: 4.1–5.1 million/µL
  • Optimal range for men: 4.5–5.5 million/µL
  • Optimal range for women: 4.2–4.9 million/µL

Why Red Blood Cells May Be Elevated

An elevated RBC count (erythrocytosis or polycythemia) may be associated with:

  • Dehydration — as plasma volume decreases, RBC concentration increases relatively
  • Smoking — the body compensates for lower oxygen levels by producing more red blood cells
  • Living at high altitude — the thin atmosphere stimulates RBC production
  • Conditions: polycythemia vera, chronic lung disease, certain kidney diseases
  • Intense physical exercise — a temporary increase after training

Why Red Blood Cells May Be Low

A low RBC count most often indicates anemia — a condition in which the blood does not have enough cells capable of carrying oxygen. Possible causes include:

  • Iron deficiency — the most common cause of anemia, especially in women
  • Vitamin B12 or folic acid deficiency
  • Chronic diseases (kidney disease, inflammatory conditions)
  • Bleeding — both obvious (menstruation, gastrointestinal) and slow, unnoticed
  • Bone marrow disorders
  • Certain medications (chemotherapy, immunosuppressants)

What to Do with Your Results

If your RBC count is slightly above or below the normal range, it does not necessarily indicate disease. Dehydration, physical activity, or the phase of the menstrual cycle may have an effect. However:

  • If RBC is below normal — it is worth checking hemoglobin, ferritin, iron, B12, and folic acid
  • If RBC is above normal — it is important to assess hemoglobin, hematocrit, and consult about further testing
  • Repeat the test fasting, in the morning, well hydrated

Monitoring Over Time

The RBC count can change for many reasons — from the menstrual cycle to seasonal variations. A single measurement captures a moment, but a trend across several months of testing provides much more information.
