What is nightly SpO2

Nightly SpO2 is the average blood oxygen saturation measured during sleep by a smartwatch or pulse oximeter. SpO2 indicates what percentage of hemoglobin molecules are carrying oxygen. In healthy adults this value is typically 95–100%. Smartwatches (Garmin, Apple Watch, Oura, Fitbit) measure SpO2 through the skin using infrared light technology — this provides an orientational measurement, not medical-grade pulse oximetry.

Normal values

The accepted threshold is 94% or above (average nightly reading). If your device consistently shows readings below 94%, this warrants a discussion with your physician. In healthy individuals, nightly SpO2 typically ranges between 95% and 98%. Brief dips during sleep (e.g., to 90%) can be normal, but frequent or prolonged drops may indicate sleep apnea.

Why nightly SpO2 may be low

Possible reasons for a consistently low nightly SpO2:

  • Obstructive sleep apnea (OSA) — the most common cause. Repeated upper airway obstruction during sleep produces periodic drops in blood oxygen levels.
  • Chronic lung disease (COPD, asthma, pulmonary fibrosis) reduces the efficiency of gas exchange.
  • Heart failure — fluid accumulation in the lungs can lower SpO2 during the night.
  • High-altitude residence — at reduced atmospheric pressure, SpO2 is naturally lower.
  • Smoking — carbon monoxide occupies a portion of hemoglobin, reducing functional saturation.
  • Anemia — although SpO2 measures the saturation percentage (not the absolute oxygen content), severe anemia can indirectly affect readings.

Why nightly SpO2 may be high

SpO2 readings above 98–100% are generally not a clinical concern. This may reflect:

  • Healthy respiratory and cardiovascular function.
  • Supplemental oxygen therapy (if in use).
  • Measurement inaccuracies — some devices may display elevated values due to algorithm characteristics.

What to do next

If your nightly SpO2 consistently falls below 94%:

1. Check measurement conditions: the watch must fit snugly, and movement can distort readings. 2. Monitor for 1–2 weeks — determine whether this is a persistent trend or isolated drops. 3.
